Colorado PERA offers an optional decreasing term life insurance program administered by Unum. In addition to paying a benefit to your survivors when you die, the life insurance program provides Accidental Death and Dismemberment (AD&D) benefits, life insurance coverage for spouses and eligible children, and an accelerated benefit option. Unum also provides free survivor financial counseling to members and their survivors.
Open enrollment occurs yearly from April 1 through May 31.
Members may enroll in the program while they are working and may keep the life insurance in retirement. Members approaching retirement are eligible to enroll during open enrollment the year prior to retirement, or in the months preceding retirement with proof of good health, called Evidence of Insurability (EOI).
Retirees are not eligible to enroll or increase coverage.
- Member premiums are paid through automatic payroll deductions.
- Retiree premiums are automatically deducted monthly from PERA retirement benefits.
Changing your Beneficiary
You can change beneficiaries at any time. Log in to your account using your User ID and password. You will see the link to the Unum website. You may also request that a beneficiary form be mailed to you by calling Unum.
In addition to the life insurance plans administered by Unum, there are several disability waiver-of-premium life insurance plans in PERA's life insurance program that are administered by PERA. If you are enrolled in one of those plans and want to change your beneficiary(ies), you may view/print the Life Insurance Beneficiary Form-Disability Waiver of Premium Policies or you may request one be mailed to you by calling PERA at 1-800-759-7372.
Filing a Claim
PERA's life insurance program also includes a number of "closed plans." These plans are closed to new enrollments, but remain in effect for those enrolled. PERA administers the New York Life, Anthem Life, and Prudential Life disability waiver-of premium closed plans, and Unum administers several other closed plans.
You may cancel Unum coverage by logging in to your account using your User ID and password. You will see the link to the Unum website. You may also cancel your life insurance by sending a written request to cancel to Unum.
