That’s why Colorado PERA offers a supplemental life insurance program administered by Unum. Unum’s decreasing term life insurance policy can supplement the coverage you may already have through your employer.

• Premiums start as low as $7.75 per month, and can be deducted from your paycheck by your employer.
• Guaranteed coverage if you enroll between April 1 and May 31 – no health questions or exams required.
• Automatic coverage for eligible dependents, at no additional cost.
• You can continue Unum coverage while you are a PERA member and into retirement.

You can enroll online by logging into your PERA account using your User ID and password, or you can print the form below and mail it to Unum.
