Life Insurance

Colorado PERA offers an optional decreasing term life insurance program administered by Unum. The life insurance program includes the following benefits:

  • Guaranteed enrollment—no health questions or exams required during the annual open enrollment period
  • Accidental Death and Dismemberment (AD&D) benefit
  • Coverage you can carry into retirement
  • Automatic enrollment for eligible dependents no additional cost
  • Affordable group rates
  • Convenient payroll deductions

Enrollment

If you are actively working for a PERA employer, you have two opportunities to enroll in guaranteed coverage:

  • Within 90 days of first becoming a PERA member.
  • During the annual open enrollment period from April 1 through May 31.

Active members may also enroll at any time during the year with proof of good health, called Evidence of Insurability (EOI).

Once enrolled, coverage can continue into retirement, though a higher monthly premium will apply. Retirees are not eligible to enroll in coverage. 

You can enroll online by logging into your PERA account using your User ID and password, or you can print the form and mail it to Unum.

There are four levels of benefits and four corresponding monthly premiums, so you can select the benefit and price that is right for you. See the Life Insurance Program Highlights brochure for more information.

Premiums for active members can be paid through automatic payroll deductions via your employer or via direct payment from your bank account. Premiums for retirees are automatically deducted from monthly PERA retirement benefit payments.

Changing Your Beneficiary

You can change beneficiaries at any time. Log in to your account using your User ID and password. In the “Accounts” drop-down menu, choose “Life Insurance.” Then, click the “Unum” link to go to the Unum website. From there, you can update your beneficiary. You may also print the form and mail it to Unum.

To file a life insurance claim, contact Unum.

Canceling Coverage

Termination of Insurance Coverage Form

You may cancel coverage by logging in to your account using your User ID and password. In the “Accounts” drop-down menu, choose “Life Insurance.” Then click the  “Unum” link. You may also send a written request to Unum to cancel your life insurance.
