Premium Information
Each employer group is individually rated for health care
premiums based on your location, the demographics of your
employees, and claims experience (if available).
To receive a premium quote, complete
the Request for Premiums
form, include any supporting data
requested on the form, and send the information to PERA
(see contact information on the form). The Request for Premiums form must be received at
PERA at least 15 business days prior to the date when the
quote is needed. PERA works directly with carriers to obtain
quotes.
Dental and vision rates are
not individually rated.
