Complete the 401(k) Plan Beneficiary Designation Form
If you are working for a PERA-affiliated employer but are not a PERA member and you would like to enroll in the PERA 401(k) Plan, you must also complete the 401(k) Participant Information Form.
DC Plan
Determine how your contributions will be invested before ING receives your first contribution to the DC Plan.

If you are working for a 457 Plan affiliated-employer but are not a PERA member and you would like to enroll in the PERA 457 Plan, you must also complete the 457 Participant Information Form.

2012 Contribution Limits
In October, the Internal Revenue Service (IRS) announced the annual maximum contribution limits for tax year 2012. In general, limits changed for 2012 to account for inflation. For the PERAPlus 401(k) and 457 Plans, the limits are as follows:

Plan Changes

Effective October 1, 2011, Colorado PERA made several changes to its 401(k), 457, and DC Plans. The 457 and 401(k) Plans are part of the new PERAPlus program. (The DC Plan remains separate.) Be sure to review the following information carefully and consider how it may affect your retirement planning and your overall financial plan.
